Complimentary Cosmetic Consultation

Patient will be asked to complete a cosmetic questionnaire. This questionnaire gives us an idea of the services that the patient is interested in.

Josie Driscoll, our Physician Assistant, will speak to the patient about their cosmetic wish list, services and products that we offer, and a general overview of prices for desired services.

Josie will take photos with the VISIA™ machine. She will print out the pictures and one copy will be kept in the patients chart and the patient receives a copy to take home.

Dr. Polisky will come in and have a face to face consultation with patient and discuss specific needs/desires of patient (fillers, peels, hair removal etc.), VISIA results and specific cost of anything discussed.

Patient can then take home VISIA print out and informational pamphlets to think about what they would like to do or…….

Patient can proceed with receiving desired services.
