Patient Forms

Cosmetic Dermatologist | Schaumburg | Elk Grove Village The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Tuesday 9:00 a.m. – 5:00 p.m. (Surgeries in office from 2:00 p.m.– 4:00 p.m.)
Wednesday 9:00 a.m. – 5:00 p.m. (Surgeries in office from 1:00 p.m. – 4:00 p.m.)
Thursday 11:30 a.m. – 7:15 p.m.(Surgeries in office from 12:00 p.m. – 2:15 p.m.)
Fridays 9:00 a.m. – 1:00 p.m.

Insurance

We accept Medicare and Blue Cross Blue Shield PPO. If you have insurance other than Medicare and BC/BS PPO we are happy to see you but ask that you pay for your visit at the time of service and we will provide you the necessary paperwork to submit the visit to your insurance company. We will file surgeries for patients with insurance other than Medicare and BC/BS PPO and ask that the patient pays 20% of the cost of the surgery at the time of service.

All surgeries are performed in office.

Patients with no health insurance are accepted at Elk Grove Dermatology. We ask that you pay for your visit at the time of service and if surgery is required we would ask for payment of 50% of the cost of the surgery at the time of service.

Cancellation Policy

We request 24 hours notice for cancellations. If you fail to provide us with 24 hours notice or you do not keep your scheduled appointment a $20.00 fee will be incurred.
